Process Overview
Who is eligible to receive services?
- People with a developmental disability—typically with a diagnosis of mental retardation—or other special needs such as mental illness or brain injury; and
- People who have approval from a funding source—federal, state, county, or private; and
- People who have completed the entire COC admission process; and
- People who do not require 24-hour nursing assistance.
To apply, follow these steps:
1. Contact the Admissions Coordinator of the region you are interested in to arrange a services assessment and schedule a tour.

2. Contact your Central Point of Coordination (CPC) for funding approval:
